Micro Format, Inc. is an approved distributor for PRINTCO INC. California controlled substance prescription forms. All California Rx formats distributed by Micro Format, Inc. are compliant with the January 1, 2020 prescription regulations.
Printed prescriptions take 10 to 12 business days for production.
All California prescriptions must be shipped signature required to the registered address of one of the providers printed on the prescription. You can choose the DEA or Medical License Address.
When ordering prescriptions, each providers DEA certificate must be emailed to custserv@rxmicroformat.com before your order can be printed.
California law requires a Physician Assistant to have their Supervising Physician's information pre-printed on their prescriptions.
Back printing is available if you need additional room for providers or addresses that will not fit on the front of the prescription.

    • The format will fit five lines of text, including the Practice Name, two providers and two addresses, or any combination of the three.
    • Back Printing is available for an additional charge if more space is needed for additional providers or addresses.
    • Each Provider's License and DEA must be pre-printed on the prescription. Please include this information in the imprint section, when placing order.
    • All prescriptions in California must be consecutively number, please select YES in the CONSECUTIVE NUMBERING box when asked.
