Micro Format, Inc. is an approved distributor for Wilmer California controlled substance prescription forms. All California Rx formats distributed by Micro Format, Inc. are compliant with the January 1, 2020 prescription regulations.
  • Printed prescriptions take 10 to 12 business days for production.
  • All California prescriptions must be shipped signature required to the registered address of one of the providers printed on the prescription. You can choose the DEA or Medical License Address.
  • When ordering prescriptions, each providers DEA certificate must be emailed to custserv@rxmicroformat.com before your order can be printed.
  • California law requires a Physician Assistant to have their Supervising Physician's information pre-printed on their prescriptions.
  • Back printing is available if you need additional room for providers or addresses that will not fit on the front of the prescription.

  • This prescription pad is 5-1/2 x 4-1/4 inches.
  • The format will fit five (5) lines of text, including the Practice Name, Two (2) Providers and Two (2) Address, Or any combination of the three.
  • This format is available as either a One-Part Script or a Two-Part Script: One-Part Pads have the secure prescription and contain 100 sheets per pad. Two-Part Pads have the secure prescription and a blank yellow carbonless copy. They contain 50 sets per pad and include a protective wrap-around cover.
  • Back Printing available for an additional charge if more space is needed for additional providers or addresses.
  • Each Providers License and DEA must be pre-printed on the prescription. Please include this information in the imprint section, when placing order.
  • All prescriptions in California must be consecutively number, please select YES in the CONSECUTIVE NUMBERING box when asked.

All prescription papers distributed SecureGuard meet the January 1, 2020 California Prescription Regulations. The California Department of Justice requires that a copy of your DEA Certificate be on file before any prescriptions are produced. Once we receive your DEA Certificate, it will be reviewed along with your order form. After it has been determined that everything is in order, your order will be processed. Any attempt to purchase prescription papers by unauthorized persons or by persons providing falsified information will be reported to the proper authorities for prosecution to the full extent of the law.
