  • This prescription is approved for use in Pennsylvania and meets all state and federal regulations.
  • This format is available as either a One-Part Script or a Two-Part Script: One-Part Pads have the secure prescription and contain 100 sheets per pad. Two-Part Pads have the secure prescription and a blank yellow carbonless copy. They contain 50 sets per pad and include a protective wrap-around cover.
  • Consecutive Numbering: Select YES if you would like your prescriptions numbered. There is an additional charge for numbering.
  • Back Print: Select YES if you need additional providers or addresses printed on the backside of the prescription. There is an additional charge for back printing.

Security Notes

Laws relative to medical prescription paper took effect on July 1, 2013. Per those regulations, ALL Medicaid prescriptions must be written on tamper-resistant paper. To comply, all Medicaid Scripts must contain one or more industry-recognized features from each of three categories of security as specified by The Centers for Medicare and Medicaid Services (CMS). Micro Format Inc., the manufacturer of SecureGuard prescription papers, produces prescription pads to meet these requirements. This site is designed for use by doctors, healthcare professionals and medical clinics. Proper documentation is required to purchase from this site and all orders are reviewed for compliance.
